Community Hospice Foundation

In 2016, Community Hospice Foundation continued to experience a transformation.  Executive Director, Monica Ojcius, and Major Gifts Officer Melissa Van Diepen, welcomed a new Administrative Assistant, Karen Aiello, and two new Donor Relations Coordinators, Shelli Prosser working here in Stanislaus County and Mary Freeman focusing her efforts in San Joaquin County. These wonderful new additions to the Foundation team allowed the Foundation to evolve to support Community Hospice in new and exciting ways.

A new campaign deemed Pick Your Passion ignited our employees and board members to personally support a fund of their choice. Additional services like an emergency food closets, help with funding the burial of a loved one or meeting a basic need is now available through this campaign to the patients and families Community Hospice serves.

An effort was well underway to have our first fundraising event in Stockton to support our growing list of patient families in San Joaquin County. Our inaugural Over the Edge event will take place in October at the start of our 2017 fiscal year. With sponsors and Edgers lining up it promises to be a great event for Community Hospice.

The Foundation submitted several grant applications for funding which bore fruit and also embarked on creating contract opportunities for area school districts to help support our growing School Grief Program.

A Case Statement identifying five initiatives for donors to tangibly offer their support was developed. The areas identified were:
  • Youth Grief: includes our School Grief Program for children and teens, Camp Erin of the Central Valley and our Chillin’ Chat 617 drop in group
  • Our Endowment Fund: is part of our planned giving opportunities allowing donors the ability to continue to support our mission beyond their life time
  • Education: investing in the continued education of our clinical staff to provide the best quality and compassionate care to patients and supporting our community through Caregiver College
  • Caring for our Patients: our promise to our patients is to care for them with kindness, dignity and respect regardless of ability to pay
  • Alexander Cohen Hospice House: our home-like, 16-bed inpatient facility provides 24-hour acute and respite care
 
The Community Hospice Foundation Case Statement was the first step in our effort to build a more robust Major Gifts Program. Additionally, we added Donor Appreciation Receptions in addition to naming opportunities at our Spyres Way office and other targeted outreach efforts. This fiscal year we realized 7 new major donors representing over $83,000 for Community Hospice.
 
Community Hospice Foundation would not thrive without the constant support and world class efforts provided by our Friends of Community Hospice. This wonderful group conducts their very own fundraising events and puts on three amazing events, Light up a Life, Fantasy of Trees, Annual Card Party and Holiday Shopping Trip. In addition they also come alongside our staff at our annual Gala and Golf tournament to drive the exciting raffles, silent and live auctions. This past year our Friends of Community Hospice were responsible for raising over $175,000 in support of our patients and their families; a banner year for which we are filled with gratitude. Hats off to President Claudia Krausnick and her fabulous Friends!

A three year Strategic Plan has been outlined and will begin implementation next year to thoughtfully execute our goals for the coming years. Our commitment to invest in our community by putting 82 cents of every dollar raised back into our mission is worthy of maintaining. Putting our donor contributions to work where they want them used is our practice, and a good one. Below you can see where our Foundation funds were directed this past year. We are using your support wisely and respectfully. Thank you for your continued generosity. Our patients and their families thank you too.
